I have known Julie Jacobs for the past 26 years and know her to be a very bright, competent, warm and empathetic individual. I have consulted with Julie often regarding my and my family's health issues and trust her medical advice implicitly.Julie has a wealth of experience in the medical field, specifically in Cardiology, Geriatric Care and Endocrinology. We love the fact that Julie takes a holistic approach to healthcare, keeping the person's emotional and physical health in mind and is very hands-on with her patients. She asks extensive questions about each individual's current care and keeps a record of all the medications her patients are prescribed, discussing the potential side effects and how they might be interacting with each other.

I have reached out to Julie as my father and mother's health failed, most recently, when our mother went into assisted living and hospice care. Julie requested copies of our mom's health records and subsequently followed up with her doctors to gather additional medical information. Upon discussing mom's health conditions, Julie adjusted and/or eliminate medications which were known to be contributors to certain health conditions mom was experiencing and informed her physicians who agreed.

With the challenges people are dealing with in not being able to get appointments with their doctors, even in the case of emergencies, I feel fortunate to know an NP and someone as knowledgeable, experienced and capable as Julie.     Here are examples of the benefits of home based medical care and avoiding hospital admission:                                                          
  • While examining a female patient with severely high blood pressure, that might typically warrant a trip to the emergency room. I stayed with her for almost two hours: I increased her medications and gave her diuretics and monitored her as her BP came down to goal level and her headache went away. Followed closely thereafter and discussed with her cardiologist.
  • A bedridden gentleman  had respiratory and cardiac issues  had very low oxygen saturations and his lungs had very little air moving. He also had very swollen legs and sacrum which are signs of congestive heart failure. By treating him immediately, with diuretics, oxygen therapy, nebulizer treatment, inhaler, we kept him out of the hospital and his condition improved.
  • I was seeing an elderly gentleman who lived alone in a single-story home. He slept in the recliner chair because he said it was just too difficult to get out of bed. I had him show me what he meant and all he needed was an under the mattress side rail so he could hold on to it to get up. I sent a picture of the device to his daughter who ordered it and the patient felt as though he had just gotten a brand-new bed! Unfortunately, I also looked at all his medications and he was taking duplicates of his diabetic medication and his hypertension medications and he often said that he was very lightheaded, we now knew why!
  • Diabetic patients tell me that they eat" healthy." I ask if I may look in their refrigerator, in their cabinets and find that they're eating very high carbohydrate foods that they were unaware of. By educating the patients and their family with the food that they have in the house they learn better and then know how to proceed appropriately. With this style of education patients' blood sugars and A1C 's are greatly improved
