I have known Julie Jacobs for the past 26 years and know her to be a very bright, competent, warm and empathetic individual. I have consulted with Julie often regarding my and my family's health issues and trust her medical advice implicitly.Julie has a wealth of experience in the medical field, specifically in Cardiology, Geriatric Care and Endocrinology. We love the fact that Julie takes a holistic approach to healthcare, keeping the person's emotional and physical health in mind and is very hands-on with her patients. She asks extensive questions about each individual's current care and keeps a record of all the medications her patients are prescribed, discussing the potential side effects and how they might be interacting with each other.
I have reached out to Julie as my father and mother's health failed, most recently, when our mother went into assisted living and hospice care. Julie requested copies of our mom's health records and subsequently followed up with her doctors to gather additional medical information. Upon discussing mom's health conditions, Julie adjusted and/or eliminate medications which were known to be contributors to certain health conditions mom was experiencing and informed her physicians who agreed.
With the challenges people are dealing with in not being able to get appointments with their doctors, even in the case of emergencies, I feel fortunate to know an NP and someone as knowledgeable, experienced and capable as Julie. I strongly recommend you schedule an appointment with her for any health issues you or your family may be encountering.
"After learning my family had a history with high blood pressure and high cholesterol, Julie encouraged me to get blood work done and immediately took action based on my unfavorable results. I had very high blood pressure for my age and needed to address it immediately. Julie prescribed medication that quickly brought me back to normalized cholesterol and BP levels and was persistent in following up on my progress. She also recommended manageable changes to my lifestyle to assist the medications in a longer-term health improvement plan. Julie frequently inquired about my comfort with the new medications and lifestyle changes by asking what I was doing and how I was feeling - providing guidance based on my answers. This gave me certainty that I was in great hands and my health was of utmost importance. I plan to see Julie for my future health matters and would highly recommend her services to my friends and family!"

"Julie Jacobs probably saved my life. I was a 78 year old obese diabetic, hypertensive, with kidney problems and COPD. In two years Julie oversaw and adjusted my medications, eating habits and mental status, I got off insulin, my A1C went from 11.1 to 6.4, my GFR from 18 to 25 and I lost 50 pounds. She coordinated care with all of my doctors, wrote prescriptions and was, and is, always available. In our home..................The best!"

"Julie has been an invaluable resource and medical advisor for me in the care of my mom. We were navigating the aging-at-home process alone until Julie gave us the ability to do this comfortably and confidently. Her holistic care approach provided the answers and knowledge to make the best decisions in the care of my loved one. She was able to advise on different medication available and techniques to help make mom comfortable. Not having to take my mom to the doctor for every UTI, illness or ache, suture or staple removal and pain has been such a blessing. I highly recommend connecting with Julie If you're looking for a medical provider that really cares for you and your loved ones and has the highest goal of improving quality of life for aging adults. She will go above and beyond to achieve this goal."

"Your Health, Your Home, Your Way."
A home-based medical practice can offer a unique and patient-centric holistic approach to healthcare that sets it apart as the best option. These are a few of the compelling reasons why.
Convenience: Patients appreciate the convenience of receiving medical care in the comfort of their own homes. This eliminates the need for travel, long waiting times in crowded waiting rooms, and the associated stress. It is particularly beneficial for elderly or disabled patients who may have difficulty getting to a traditional clinic, as well as allows the provider to see how the patient is really functioning in their home.
Personalized Care: Home-based medical practices allow for more personalized care. Healthcare providers can take the time to understand a patient's living environment, lifestyle, and specific needs, leading to tailored treatment plans and a stronger doctor-patient relationship. Ability to make assessments for care that may not have been addressed previously.and gain a more comprehensive view of their patients' daily lives. This facilitates better long-term management of chronic conditions and the prevention of hospital readmissions
Reduced Risk of Exposure Especially in times of health crises, like the COVID-19 pandemic, home-based medical practices offer a safer option as they minimize the exposure to germs and infections commonly found in healthcare facilities.
Improved Communication: Home visits allow for more meaningful and open communication between patients and healthcare providers. Patients may feel more comfortable discussing their concerns, and providers can better educate patients about their conditions and treatment options.
Flexibility: Home-based practices can often offer more flexible scheduling, including evening and weekend appointments, which can accommodate patients with busy lives or irregular work schedule.
Family Involvement: Home-based care encourages family involvement in the patient's healthcare journey. This can provide emotional support, help with medication management, and ensure that the patient receives holistic care.
I approach patient care holistically, addressing emotional, spiritual, mental, physical, and environmental factors. This approach emphasizes balance between these aspects, recognizing the close connection between environmental and physical health. As a Nurse Practitioner with Full Practice Authority from the Illinois State Board of Nursing, I assess patients comprehensively, involve them in decision-making, and provide education to optimize their well-being. I have the ability to diagnose, order tests, prescribe medications, manage treatment modalities and educate.
